Language Assistance Services - IDPH

If you believe IDPH has failed to provide the services noted above, denied you the benefits of the IDPH Language Access Plan, or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, a grievance can be filed with the IDPH equal employment officer at 312-814-1041. Grievances can be filed in person or by mail or email at:

A written complaint must be filed within six months of the alleged denial. If you need help filing a grievance, staff assistance is available  through the IDPH Office of Finance and Administration (217-782-5934) or IDPH Center for Minority Health Services (217-785-4311).

Not satisfied with the findings or corrective action taken in response to your complaint?  You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office of Civil Rights Complaint Portal or by email, mail, or phone at:
